Purpose of the test

The statin check-up panel evaluates how well your body is responding to treatment in lowering your LDL cholesterol and determines whether the statins are adversely affecting your liver and muscles.

For some individuals taking statins, blood sugar levels can increase, which may lead to type 2 diabetes, a risk significant enough that the FDA has issued a warning on statin labels. Metabolic Markers

Comprehensive Metabolic Panel (CMP): 14-component panel measuring:

Complete Urinalysis (UA): 12-component panel measuring:

  • Color: The visible color of your urine, a basic indicator of hydration and health.
  • Appearance: Whether your urine is clear or cloudy, which can reflect cells or particles.
  • Specific gravity: How concentrated your urine is compared to pure water.
  • pH: The acidity or alkalinity of your urine; a high pH may indicate a urinary tract infection (UTI) or kidney issues, while a low pH is seen with diabetic ketoacidosis and diarrhea
  • Protein: Detects protein in urine, which isn’t normally present in significant amounts.
  • Glucose: Detects sugar in urine, which can appear when blood glucose is high.

  • Occult blood: Detects blood in urine (hematuria) that may not be visible to the eye.

Before the test

Fast for 9 to 12 hours before your draw. Water is fine. Fasting keeps your triglyceride levels accurate because the lab uses triglyceride levels to calculate your LDL cholesterol.   • Skip food and caloric drinks for 9 to 12 hours. Water is allowed.
  • Avoid alcohol and high-fat meals the evening before. Both can raise triglycerides overnight.
  • Take your statin as prescribed on the morning of the test. The panel measures whether the medication is working at your usual dose. Skipping it would make LDL look lower than it normally runs, which could mask a needed dose change.

During the test

  • Bring a photo ID and your order confirmation. Check in at the patient service center.
  • A phlebotomist finds a vein in your arm, usually at the inside of the elbow. You’ll feel a brief pinch and mild pressure during the draw.
  • One or two tubes of blood are collected. The whole draw takes five to 10 minutes.
  • A small bandage covers the site. Keep it on for at least 15 minutes and avoid heavy lifting with that arm for a few hours.

Most people feel nothing after the draw. Light bruising over the next day or two is normal.

Don’t wait for your next scheduled test if you develop muscle pain, weakness, or dark-colored urine after starting or changing a statin. Contact your provider right away. These can be early signs of myopathy.

After the test

Results are typically ready within one to three business days after the lab receives your sample, though timing can vary by lab. They’ll come through your ordering provider’s patient portal or office. Some providers review results before releasing them. If you don’t see results within three business days, call the provider’s office.

What Do My Results Mean?

Your results show two things: whether each biomarker falls within a healthy range, and whether your LDL cholesterol has reached the ACC/AHA target for your risk category. Reference ranges vary slightly between labs. Your provider reads your results in the context of your full cardiovascular history, not the numbers alone. The American College of Cardiology (ACC) and the American Heart Association (AHA) have set guidelines to prevent a first heart attack or stroke: those who are borderline or at intermediate risk should have an LDL-C level less than 100 mg/dL, and for high-risk individuals, the goal is less than 70 mg/dL. And the goal for very high-risk individuals with ASCVD (atherosclerotic cardiovascular disease) is less than 55 mg/dL.

If your results are within range

LDL-C has reached your ACC/AHA target. The statin may be working as intended. If liver enzymes (ALT and AST) are within range, it means statin-related liver stress has been avoided. If HbA1c is within range, it means blood sugar is under control. Schedule your next check per your provider’s guidance, typically every three to 12 months based on your cardiovascular risk, and keep up the habits that support the result.

If your results are out of range

LDL still above target: The statin may not be at the right dose, or dietary factors may be blunting the effect. Your provider may adjust the dose, switch statins, add ezetimibe or a PCSK9 inhibitor, or look at adherence and diet. Don’t adjust your medication on your own.

High ALT or AST: Occasionally, statin use causes an increase in liver enzymes. If the increase is mild, you can continue to take the medicine. Low to moderate doses of statins don’t appear to raise liver enzyme levels severely. If ALT and/or AST levels rise above three times the upper limit of normal, your provider will probably pause or switch the statin therapy to allow the liver to recover. If there are signs of jaundice, fatigue, nausea, or dark urine, the statin should be immediately stopped. Enzyme levels need to be rechecked before restarting.

High CK: Mild elevation (less than four times the upper limit of normal) with no symptoms is usually monitored, but not acted on right away. If CK is 10 times the upper limit of normal, especially if you have symptoms of rhabdomyolysis, stop taking your statin and seek medical treatment right away, as your provider can take steps to help prevent kidney damage and other complications.

Triglycerides above 400 mg/dL: Your LDL-C result may not be reliable due to the fatty consistency (triglycerides) of your blood that interferes with the test. Your provider will interpret with caution or order a direct LDL cholesterol test instead.
